COVID-19 Vaccines and Boosters
If you need your first, second or booster dose of the Pfizer COVID-19 vaccine, please visit one of our UofL Health – Urgent CarePluslocations. Please visitUofLHealth.org/Urgent-Careto schedule your COVID-19 vaccination at any of our five convenient locations.
COVID-19 Booster Shots for UofL Health Team Members Who Received the Pfizer Vaccine
Those who work in high risk occupations such as health care may receive a booster dose.With the changing landscape of COVID-19 booster shots, UofL Health is not mandating booster shots at this time. UofL Health team members who received their first two doses of the Pfizer vaccine more than six months qualify for the COVID-19 booster shot.
When it has been at least six months since you received your second dose of the Pfizer vaccine, you will qualify for a booster shot. For example, if you received your second dose of the Pfizer vaccine before April 2021, you will be immediately eligible.
If you qualify for the booster, you can make an appointment at one of our Urgent CarePluslocations.
What should I know before I get my booster?
Team members should bring their original COVID-19 vaccination cards to their appointment so their booster dose can be documented appropriately. It is also a good idea to consider your reaction to the second dose when scheduling your booster. Most patients are experiencing a similar, or sometimes lesser, reaction to the booster as the second dose.
What To Do If You’ve Lost Your Vaccine Card
If you’ve lost your vaccination card, there are resources you can use to obtain your proof of vaccination.
To find out if your immunization records are in the Kentucky Immunization Registry, please complete theKYIR Release of Immunization Record and fax it to the registry’s helpdesk at 502-564-4760.
The COVID-19 card you received at the time of vaccination is issued by the CDC. It cannot be replaced with another card.

Pfizer Vaccine Earns FDA Approval
As of Aug. 23, 2021, the FDA has provided full approval of the Pfizer COVID-19 vaccine. This is a monumental announcement as we push to get more Kentuckians and our staff vaccinated against COVID-19. As a condition of employment and medical staff privileges, everyone will need to show proof of at least initial vaccination by Sept. 15, 2021. Vaccines can be scheduled at UofLHealth.org/Louisville-Covid-19-vaccinations/. Also, if you have had a vaccine, please be available for questions from our staff who may not have yet gotten vaccination and be a champion for this life-saving intervention in your neighborhoods and communities.

UofL Health Emergency Department visitor policy update (Aug. 17, 2021)
The last few weeks have been difficult ones regarding COVID-19 for many communities across the United States. We continue to see an increase in the number of positive cases, and we have seen a significant spike in hospitalizations across our region of the US. Within Kentucky this week, we have seen a doubling in the total number of positive COVID cases, and, as a result, more and more patients are seeking treatment at our Emergency Departments and Urgent Care Plus locations. This high number of visits and increasing numbers of patient holds in our emergency departments has led to crowding in these areas making patient care difficult and putting patients and staff at risk as social distancing has become problematic.
Due to these issues, we will be closing UofL Health Emergency Departments and Urgent Cares to visitors beginning today (Aug. 17). Specific policies on how we will communicate with our patients’ families and designated contacts are attached to this email. Additionally, we will still allow a guardian or parent to accompany all pediatric patients and will allow nursing staff to make accommodations for a visitor under extreme circ*mstances.
We sincerely hope these changes will be temporary, and we are not changing our current visitation policy for our inpatient facilities. We appreciate your patience as we continue to provide care to this fourth wave of the pandemic.
